1. 情绪低落三部曲——绝望、无助、无用
处于抑郁状态下的人往往容易对前途感到悲观失望,看不到希望;继而出现孤立无援的感觉,认为自己已经无药可救(这一般都是自己一厢情愿的想法);然后就开始否认自己、批判自己,认为自己一无是处。这种绝望、无助、无用形成一种恶性循环,让人难以自拔,这时若一味只求自救,往往只会事倍功半。
2.兴趣缺乏与乐趣丧失是怎么回事儿?
兴趣缺乏是对各种以前喜爱的活动缺乏兴趣,比如以前是户外运动爱好者,现在却对朋友的邀约百般推脱,毫无兴致;有时工作之余整天躺在床上,死气成成;更有甚者,离群索居,不愿见人。而乐趣丧失,则是快感缺乏,体验不到快乐。表面上在做以前爱干的事情,但总觉得不对劲、没有劲,有时甚至为了找回以前的快感而反复体验。是为了消磨时间,或是为了从悲伤走出来,但这种机械地、完成任务似的重复经常难以重拾旧梦,因为抑郁剥夺了我们感受幸福的能力。
心理症状群
1.焦虑,我该拿你怎么办?
焦虑与抑郁就像是一对孪生兄弟,你中有我,我中有你。当抑郁状态长期得不到缓解时,焦虑就慢慢找上门来了,不过你还不一定能认出他来。主观的焦虑症状大家都体验过:坐立不安、心慌焦躁;如果出现躯体症状呢(头晕、胸闷、尿频、出汗等)?大多数人都会把他错认为其他毛病,像更年期综合症、心肌缺血、脑血管硬化等。所以说,反复出现某种特别的症状,又不能用其他疾病解释时,尝试看看焦虑抑郁门诊吧!

常常有抑郁症患者抱怨“脑子跟进了水一样”、“脑子像是没有上润滑油”、“脑子开不动了”“脑子如浆糊一般”。注意力与记忆力下降是抑郁症典型症状之一,而与之相随的是运动迟缓、工作效率下降,严重的可以达到木僵的程度,就跟不语不动的“木头人”一样。
有一个有趣的说法是:健康时想问题能翻来覆去的想,正着一个圈,反过来又一个圈,一旦抑郁了,这个圈就卡住了,正着都画不完。而与之相反,有些“变异”的抑郁症患者也可能出现烦躁不安、紧张激越,他们经常不知自己因何烦恼,却又无法自控。曾有一名情绪控制不好、脾气暴躁的抑郁患者,家人很难接受他患病的事实,有时甚至认为他是无病呻吟;他的苦衷难以被理解,他说“我的脑袋里面全是一些漫无目的的思绪,毫无条理却又不能控制;整个人都很紧张,所以一点小事就能把我惹毛。我知道不能这样,但是,我用尽方法,就是控制不住自己。我真的尽力了,你相信吗?”
不管是脑子开不动也好,紧张烦躁也罢,思流的阻滞或激荡等等,这些总能给抑郁症患者带来的烦恼。
躯体症状群
躯体症状群是一大类非特异性症状的集合,包括睡眠紊乱、食欲紊乱、性功能问题、精力丧失、晨重夜轻、疼痛、胃肠功能紊乱等。
什么是睡眠紊乱?比较简单的分法,我们将睡眠分为好几个阶段。早段,即入睡阶段,若上床之后辗转反侧不能入睡超过30分钟以上(当然,不包括看手机玩微信的时间,即从真正开始睡觉算),就算是入睡困难了。中段,即睡眠保持阶段,若睡眠过浅,容易反复醒来则会直接导致睡眠时间及睡眠质量不足。末段失眠,即早醒,是抑郁患者具有有特征性的症状,早醒后往往难以再次入睡,白天的精神状况可想而知。与这些典型表现不同的是,在不典型抑郁患者可以出现贪睡的情况。
食欲紊乱有什么表现?食欲下降与体重减轻是具有代表性的症状,当然还有一些“特立独行”的表现为食欲亢进与体重增加。食欲减退的发生率为70%左右,轻者食不甘味,体重不一定出现明显变化,而重者甚至出现亚木僵状态无法进食。
懒言少动,无精打采是“肾虚”吗?经常在门诊看到抑郁患者的家属孜孜不倦地请教“请问下怎么‘补肾’呢?他可能是‘肾虚’,老是说没劲,比以前懒多了,也不出去玩,整天呆家里变得越来越没精神了”。小编想告诉大家的是,抑郁症患者并不是因为不愿出门、老呆家里才“抑郁”的,他们是因为生病了,患了“抑郁症”才会变得没精力、没动力。抑郁症并非他们所想、所选,不要问“你为什么会抑郁?”,不要责难“你怎么这么懒?”,更不要放弃“烂泥扶不上墙,我再也不想管你了!”,因为是抑郁先选择了他们。
没有心理症状,只有躯体症状还算抑郁吗?曾经有很多无辜的病人因为各种躯体症状长期游荡在综合医院的各个门诊,疼痛、周身不适、胃肠功能紊乱、心慌气短……刚开始家属可能还能理解,积极带其求治,到后来焦急变成焦心,焦心变成心烦,心烦变成烦躁,所有的耐心消失殆尽。我们要知道,抑郁症的外部表现非常复杂,非专业人士即便是非精神科的医师也很难马上从一些蛛丝马迹中找到真相。非精神科医生往往认为是“胃肠神经功能紊乱,心脏神经官能症,更年期综合征”等等。
上面提到的各种核心及心理症状固然是抑郁症的典型症状,但是,很多时候抑郁症也可能通过躯体症状表现出来。或许由于我们的含蓄,我们的不善于表达情感,躯体的不适成为了情绪的一个信号。所以说,千万不要因为只有躯体症状来否认其患抑郁症的可能性。当然,也不能杯弓蛇影,稍有躯体不适,就陷入抑郁的恐惧之中。
抑郁的三大武器——核心症状、心理症状群、躯体症状群,让大家见识到了他变脸的功底,同时,也让我们认识到抑郁的反面,并非只是快乐,还有活力、乐趣、淡定、睿智……有一名抑郁症患者,曾生动地描述过他的抑郁:“我突然发现自己对几乎所有的事情都失去了兴趣,甚至不愿意去做那些我曾经很想去做的事情,但是我不知道为什么。
所有需要完成的事情,都感觉那么麻烦。有时该吃午饭了,我却开始想,我还得把食物拿出来,放到盘子里,得切,得嚼,得咽!”如此日常的生活在他们眼中变得如此复杂而又困难,这也许超出了你的想象范围,但是,这就是他们,一群现在与抑郁作斗争,也许将来的每都将与之抗争的人类!请带着一点宽容,抛掉一些品评,真心实意地接纳他们、帮助他们!
